New York: Harper &amp; Brother.\u00a0<strong>Permissions:<\/strong> Public domain.<\/span><\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<h1>Les d\u00e9buts de la radioprotection<\/h1>\n<p>En d\u00e9cembre 1895, Wilhelm Conrad R\u00f6ntgen (Allemagne) d\u00e9couvre les rayons X, et en novembre 1896, Henri Becquerel (France) remarque la radioactivit\u00e9 de l\u2019uranium. \u00c9mil Herman Grubb\u00e9 (\u00c9tats-Unis), un fabricant de tubes de Crookes[<a href=\"#_ftn2_FR\" name=\"_ftnref2_FR\">2<\/a>] situ\u00e9 \u00e0 Chicago, utilisait son \u00e9quipement pour \u00e9tudier la fluorescence des mat\u00e9riaux d\u00e8s 1896. En janvier de cette m\u00eame ann\u00e9e, Grubb\u00e9 remarque qu\u2019il a d\u00e9velopp\u00e9 un \u00e9ryth\u00e8me, une ulc\u00e9ration, puis une cicatrisation des mains. Notant les possibles aspects th\u00e9rapeutiques des rayons X, Grubb\u00e9 traite m\u00eame une patiente atteinte d\u2019un cancer du sein \u00e0 la fin de janvier 1896 (Grubb\u00e9, 1933).<\/p>\n<p>En f\u00e9vrier 1896, John Daniel et William Lofland Dudley de l\u2019Universit\u00e9 Vanderbilt (\u00c9tats-Unis) radiographient la t\u00eate de Dudley et produisent ainsi la premi\u00e8re radiographie diagnostique (Sansare et al., 2011). En juillet 1896, Victor Despeignes (France) d\u00e9clare utiliser des rayons X pour traiter le cancer de l\u2019estomac (Leszczynski et Boyko, 1997).<\/p>\n<p>En mars 1897, Stone Scott (\u00c9tats-Unis) rapporte 69\u00a0cas de l\u00e9sions cutan\u00e9es caus\u00e9es par les rayons X (Scott, 1897). Il est \u00e0 noter que la toute premi\u00e8re poursuite pour br\u00fblures par rayonnements a \u00e9t\u00e9 d\u00e9pos\u00e9e en 1899 (Grand Forks, 1899).<\/p>\n<p>Avant que Bergoni\u00e9 et Tribondeau (France) \u00e9tudient les changements cellulaires provoqu\u00e9s par l\u2019exposition aux rayons X dans les testicules de rats en 1904 (Vogin et Foray, 2012), Heineke (Allemagne) provoquait une leuc\u00e9mie chez les rats apr\u00e8s exposition aux rayons X en 1903 (Heineke, 1905).<\/p>\n<p>Il est clair qu\u2019au cours de la premi\u00e8re d\u00e9cennie suivant la d\u00e9couverte des rayons X et de la radioactivit\u00e9, plusieurs choses se passent dans le monde pour \u00e9tudier les effets de l\u2019exposition aux rayonnements ionisants.<\/p>\n<p>En Allemagne, vers 1911, Otto Hesse recueille les ant\u00e9c\u00e9dents de 94\u00a0cas de tumeurs radio-induites (dont 50 cas chez les premiers radiologues) (Hesse, 1911). En 1922, Ren\u00e9 Ledoux-Lebard (France) estime que plus de 100\u00a0des premiers radiologues sont morts d\u2019un cancer d\u2019origine professionnelle (Patterson, 1973).<\/p>\n<p>Le premier d\u00e9c\u00e8s d\u00e9clar\u00e9 d\u2019une travailleuse de l\u2019industrie du cadran au radium remonte \u00e0 1925 (Rowland, 1944). En 1939, Sigismund Peller (\u00c9tats-Unis) signale 47 cas de cancer du poumon chez 400 mineurs travaillant dans les mines de pechblende[<a href=\"#_ftn3_FR\" name=\"_ftnref3_FR\">3<\/a>] de Joachimsthal en Boh\u00eame (CNR, 1988). En effet, la science de l\u2019\u00e9pid\u00e9miologie, introduite au milieu des ann\u00e9es 1800 pour les maladies infectieuses, a commenc\u00e9 \u00e0 \u00eatre utilis\u00e9e pour \u00e9tudier l\u2019exposition \u00e0 des agents non infectieux, tels que l\u2019exposition aux rayonnements.<\/p>\n<p>La formalisation de la r\u00e9glementation en radioprotection a commenc\u00e9 environ une quinzaine d\u2019ann\u00e9es apr\u00e8s la d\u00e9couverte des rayons X! En effet, la Soci\u00e9t\u00e9 radiologique allemande a \u00e9tabli des normes en 1913 en fonction des doses d\u2019\u00e9ryth\u00e8me cutan\u00e9 (Stabin, 2007). En 1915, la <em>British Roentgen Society<\/em> commence \u00e0 formuler des recommandations en mati\u00e8re de radioprotection (Meinhold, 2004). De nombreux endroits \u00e9tablissent alors leurs propres normes en fonction des doses d\u2019\u00e9ryth\u00e8me cutan\u00e9 mesur\u00e9es pour leurs \u00e9quipements (g\u00e9n\u00e9ralement de 300 \u00e0 600\u00a0rads) (Inkret et al., 1995).<\/p>\n<p>Ainsi, en 1921, la <em>British Society<\/em> cr\u00e9e le Comit\u00e9 des normes de protection contre les rayons X et le radium, qui a publi\u00e9 des recommandations entre 1921 et 1948, \u00e0 mesure que des connaissances suppl\u00e9mentaires sur la radioprotection \u00e9taient acquises. Par exemple, les normes de protection britanniques de 1921 recommandaient\u00a0:<\/p>\n<p style=\"padding-left: 40px;\">(i) Pas plus de sept heures de travail par jour,<br \/>\n(ii) Les dimanches et deux demi-journ\u00e9es de cong\u00e9 par semaine,<br \/>\n(iii) Passer autant de temps que possible \u00e0 l\u2019ext\u00e9rieur,<br \/>\n(iv) Un cong\u00e9 annuel d\u2019un mois ou de deux quinzaines, et<br \/>\n(v) Les religieuses et les infirmi\u00e8res travaillant \u00e0 temps plein dans les services de radiologie et de rayons X ne devraient pas \u00eatre sollicit\u00e9es pour d\u2019autres services hospitaliers. (Meinhold, 2004)<\/p>\n<p>Au milieu des ann\u00e9es 1920, Arthur Mutscheller (\u00c9tats-Unis) \u00e9tablit les premi\u00e8res limites de dose de tol\u00e9rance quantitative. Il fixe une limite d\u2019un centi\u00e8me de la dose d\u2019\u00e9ryth\u00e8me cutan\u00e9 sur une p\u00e9riode de 30 jours, soit une dose de 72 rads par ann\u00e9e (Inkret et al., 1995). Les premiers radiologues recevaient en moyenne 50 rads (500 mGy) par ann\u00e9e. Comparez ces valeurs aux limites maximales actuelles pour les travailleurs de 50 mSv au cours d\u2019une ann\u00e9e donn\u00e9e, avec un maximum de 100 mSv sur une p\u00e9riode de cinq ans, \u00e9tablies par la Commission canadienne de s\u00fbret\u00e9 nucl\u00e9aire (CCSN)[<a href=\"#_ftn4_FR\" name=\"_ftnref4_FR\">4<\/a>]!<\/p>\n<p>En 1929, les \u00c9tats-Unis cr\u00e9ent le Comit\u00e9 consultatif sur la protection contre les rayons X et le radium (<em>Advisory Committee on X-ray and Radium Protection<\/em>, ACXRP) et leurs premi\u00e8res recommandations en 1931 sugg\u00e9raient une limite de 0,2 R[<a href=\"#_ftn5_FR\" name=\"_ftnref5_FR\">5<\/a>] par jour (50 rads par ann\u00e9e). Cette limite a \u00e9t\u00e9 r\u00e9duite \u00e0 0,1 R par jour (25 rads par ann\u00e9e) en 1936 (Stabin, 2007).<\/p>\n<p>Pour aider \u00e0 quantifier l\u2019exposition, des dosifilms et des chambres d\u2019ionisation sont d\u00e9velopp\u00e9s dans les ann\u00e9es\u00a01920. Robert Landauer utilisait du film de radiographies dentaires pour estimer les limites de tol\u00e9rance de l\u2019\u00e9ryth\u00e8me cutan\u00e9, et Victoreen a fabriqu\u00e9 son premier <em>Condenser R-Meter<\/em> en 1923, permettant de mesurer quantitativement les rayonnements (Taylor, 1981).<\/p>\n<p>En 1941, l\u2019ACXRP am\u00e9ricain \u00e9tablit la charge corporelle maximale admissible pour le radium \u00e0 0,1\u00a0\u00b5Ci (ou 0,1\u00a0\u00b5g) (Rowland, 1994).<\/p>\n<p>Le premier r\u00e9acteur nucl\u00e9aire (la <a href=\"https:\/\/www.newyorker.com\/tech\/annals-of-technology\/remembering-chicago-pile-worlds-first-nuclear-reactor\" target=\"_blank\" rel=\"noopener noreferrer\">Chicago Pile<\/a>, article en anglais seulement) est construit \u00e0 l\u2019Universit\u00e9 de Chicago et on le teste le 2 d\u00e9cembre 1942, et le domaine de la radioprotection moderne est commenc\u00e9. (Le terme \u00ab physique de la sant\u00e9 \u00bb pour <em>Health Physics<\/em> a \u00e9t\u00e9 propos\u00e9 pour masquer le fait, pendant la guerre, qu\u2019il s\u2019agissait de la science de la radioprotection.)[<a href=\"#_ftn6_FR\" name=\"_ftnref6_FR\">6<\/a>] Le Conseil national de radioprotection (<em>National Council on Radiation Protection<\/em>, NCRP) est fond\u00e9 \u00e0 partir de l\u2019ACXRP en 1946 (Stabin, 2007). Pendant ce temps, au Canada, la Commission de contr\u00f4le de l\u2019\u00e9nergie atomique (CCEA) est aussi fond\u00e9e en 1946, et devient la CCSN[<a href=\"#_ftn7_FR\" name=\"_ftnref7_FR\">7<\/a>] en l\u2019an 2000.
